1. 개요

설사약(泄瀉藥)은 설사에 관한 조절을 하는 약 전반을 일컫는 말이다. 특이하게도 '설사를 멈추는 약'인 지사제(止瀉劑, diarrhea remedy/anti-diarrheal)와 '설사를 일으키는 약'인 사하제(瀉下劑, laxative/purgative/aperient), 정반대의 효과를 갖는 이 두 가지 약이 동시에 '설사약'으로 불리고 있다. # 따라서 약국에서 약을 구매할 땐 특히 주의해야 한다. 풀어서 말하거나 지사제·사하제라고 부르자. 아니면 의사에게 처방을 먼저 받자. 물론 약사 쪽도 이걸 알기에, 설사를 멈추게 하는지/변비를 해소하기 위한지, 먼저 어느 쪽이 필요 하냐고 물어본다.

설사약이 정 반대의 두 의미를 다 지닌 이유는, 심한 설사를 멈추게 만들어야 하는 경우와, 심한 변비 해결이나 장청소를 위해 일부러 설사를 일으켜야 하는 경우가 모두 있기 때문이다. 반면 변비약은 변비를 치료하는 약만 의미하는데, 의학적으로 일부러 변비에 걸리게 해야 할 경우가 없기 때문이다. 하지만 대체로 일상생활에서 설사약이라 불리는 경우는 '설사를 멈추게 하는 약' 으로 쓰인다. 감기약이 감기를 일으키는 약이 아니라 감기를 낫게 하는 약이라는 것을 생각해 보면 이해가 쉽다.

장 운동 정상화를 위해 진경제를 쓰기도 하며, 이외에도 '트리메부틴' 성분.[1] 의 장 운동 정상화제나, 스멕타이트 성분[2] 의 광물 기반 지사제(유해성분 흡착제)를 쓰기도 한다.

2. 지사제

지사제(止瀉劑, diarrhea remedy/anti-diarrheal)는 설사를 멎게 하는 약이다. 설사가 지속적으로 반복되면 불편이 심하며 탈수 등의 심각한 문제를 초래하므로 설사를 멈추는 용도의 약이다. 주의할 점은 이 약은 설사를 무조건 멈춰버리는 약[3]이어서 함부로 사용해서는 안 된다는 것이다. 설사의 원인이 만약 감염이나 독소에 의한 것인데, 무작정 지사제를 사용하면 장운동 자체가 멈춰버려 세균과 독소까지 배출이 되지 못해 장 내에 머물되며 응축되고, 결국 안 먹느니만 못한 상황에 처할 수 있다.[4] 그래서 병원에서 지사제를 처방해 줄 때는 원인을 먼저 따져보고, 유산균/정장제/항생제 등을 같이 처방해 줄 때가 대부분이다. 의사처방 없이 약국에 가서 바로 살 때도 약사와 상의하는 것이 좋다.

저격수는 한곳에 오래 있어야 하여 배변활동을 그자리에서 해결해야 한다는 특수성이 있어서 먹기도 하지만 주된 이유는 분뇨의 냄새에 의한 군견들의 반응으로 위치와 이동경로의 발각을 막기 위해서이다.

2.1. 설사지연제

지사제와 달리 설사가 나오는 것을 조금 늦춰주어 화장실에 갈 시간을 버는 용도이다. 일본의 '스토퍼 설사방지약 EX'(ストッパ下痢止めEX)'이 가장 유명하며 국내 지사제 중에도 비슷한 역할을 하는 것이 있으니 약국에서 물어보고 사두자.

다음은 스토퍼 설사방지약 EX에 대한 설명이다.

3. 사하제

사하제(瀉下劑, laxative/purgative/aperient), 줄여서 하제(下劑) 또는 사제(瀉劑)는 창자의 내용물을 배설시킬 목적으로 사용되는 약제이다. 쉽게 말해 의도적으로 설사를 유발시키는 약이다. 작용의 강약에 따라서 약한 것은 완하제(緩下劑), 강한 것은 준하제(峻下劑)라고 한다. 표준어는 아니지만 우리말샘에 수록된 용어로 연하제(軟下劑)도 있다. 변을 연(軟)하게 만들어 배출시키는 하제라는 뜻이다. 소르비톨 등의 당알코올은 대표적인 사하제다.

사하제라는 이름이 잘 쓰이지 않기에 오해하는 사람도 가끔 있지만, 사실 사하제의 다른 말은 변비약이다. 다만 서로 반대인 두 가지 뜻을 가진 설사약과는 달리 변비약은 변비를 발생시키는 약의 뜻으로 쓰이지는 않는다.

먹으면 설사를 유발하는 음식들도 여럿 있으며, 곤약, 기름치 등이 있다. 기름치는 참치랑 비슷해서 가짜 참치로 꽤나 유통되는데, 대부분의 사람들이 기름치를 먹으면 설사를 한다. 곤약은 주성분인 글루코만난이 일종의 사하제 역할을 한다.

유당불내증이 있는 사람이 우유를 마시면 같은 효과가 난다.

보통 배변을 잘 보지 못해 배가 아프거나 일부 과식을 해서 혹은 다이어트를 위해 장을 비우고자 먹는 경우가 많다. 하지만 뭐든 적당히가 좋다는 말이 있듯 심각한 변비나 일부 의료적인 이유가 아니라면 억지로 설사를 유도하는 것은 약이든 민간요법[6]이든 몸에 좋지 않다. 그러므로 건강한 식단[7]과 적당한 운동으로 장 건강을 챙기는 것이 중요하다고 볼 수 있다.

변비를 해소할 목적의 하제가 있는 반면, 대장 내시경을 위해 장 내부를 모조리 비우기 위한 하제도 있다. 이 경우는 과립형, 또는 알약 형태의 하제[8]를 복용한 뒤 1~2리터의 물을 시간차를 두고 천천히 음용하는 식으로 이루어진다. 물론 이 약을 복용할 경우 말 그대로 폭풍설사가 폭포수처럼 이루어지며, 설사가 투명해질 때까지 계속 나오게 된다. 약을 먹기 전 금식은 필수. 만니톨이 오랫동안 관장약으로 사용되었으나 1977년 여름 프랑스에서 만니톨로 인해 대장이 폭발하는 일명 '대장폭발' 사고가 일어난 후로 관장약으로는 쓰이지 않는다. 낭시에 있는 한 대학병원 소화기내과에 69세 남성이 대장에 용종이 생겨 용종절제술을 받기 위해 찾아와 의사가 내시경을 통해 전류를 흘려보내 용종을 태우기 위해 전류의 세기를 4에 맞춰놓고 전류를 흘려보냈는데 8초 후 장내가스가 폭발하며 환자는 머리 쪽으로 검사대에서 튕겨 나왔고 내시경 장비는 환자의 몸 밖으로 어뢰처럼 튀어 나왔다. 이에 대수술을 진행했으나 결국 환자는 과다 출혈로 사망하고 말았다. 의료사고 원인을 조사한 결과 만니톨로 관장을 했는데 환자의 대장은 이미 내용물을 싹 비운 상태였지만 세균이 아직 남아 있어서 세균이 만니톨과 반응해 수소를 많이 생성했고 그때 전류를 흘려보내며 세균이 생성한 수소가 산소와 반응해 물을 만들며 엄청난 양의 에너지가 방출돼 폭발을 한 것이다. 이 사건 이후 만니톨은 더 이상 설사약(사하제)으로는 쓰이지 않으며[9] 관장을 할 때는 내용물뿐만 아니라 세균과 가스까지 남김없이 다 빼내게 되었다.

또한 시럽병의 입구가 그리 크지 않은 관계로 가루약이 시럽병 옆으로 흩어지는 문제가 되기 쉬우므로, 시럽병에 들어갈 수 있는 깔때기 역할의 좁은 약포지를 쓰는 게 좋다.

장난으로 설사약 50개가 들어간 브라우니를 먹고 급성 횡문근융해증으로 고통받은 중학생의 사례가 유튜브에서 소개되었다. 해당 중학생은 설사약이 든 브라우니를 먹고 변의를 참는 내기를 하느라 설사약을 과량 복용했는데, 해당 설사약은 센나글리코사이드(sennocides) 종류로, 과량 복용으로 인해 수분이 장으로 이동하게 되고, 전해질 불균형이 일어나 몸에 칼륨이 부족해졌다. 칼륨 부족으로 근육이 이완되지 않고 경련한 끝에 횡문근융해증이 발병한 것이다.

분뇨 처리장, 하수처리장, 하수관거, 정화조 등 개인하수처리시설, 하천에서 검출되기도 한다.

4. 지사제와 사하제를 같이 먹으면

▶변비약이랑 설사약을 동시에 먹으면 어떻게 될까?

지사제와 사하제는 약물의 종류에 따라 작용 기전이 다르고 약효의 강도가 다르므로 어느 약을 선택하느냐에 따라 결과는 다를 수 있다. 사하제는 팽창성, 삼투압성, 자극성, 상피기능변용약, 소화관연동운동활성약 등이 있고, 지사제는 수렴제, 흡착제, 장운동억제제 등이 있다. 특히 의료인의 진단 처방 없이 장운동억제제를 임의로 복용할 경우 장마비가 올 수 있으므로 위험하다.

산과 염기의 중화 반응을 떠올리고 같이 먹으면 아무 일도 없을 거나 증상이 약할 것이라고 생각하는 경우가 많은데, 그렇지 않으며 상당히 위험한 행동이다.

간단히 요약하자면 총을 쏘는데 총구에다가 이물질을 껴놔서 막아놓은 상태라고 보면 된다. 총이 터지거나 망가질 것이 뻔하다.

5. 매체

창작물에서는 싫어하는 사람에게 사하제를 먹여 설사를 하게 하는 용도, 혹은 사하제를 먹여 방해되는 상대가 자리를 비우게 하는 용도로 묘사되는 경우가 많다.

6. 여담

[1] 트리메부틴 성분은 다른 진경제와 작용 기전이 다른다.[2] 알루미늄, 마그네슘, 규소 등을 포함한 점토이다. 이 점토는 물, 박테리아, 바이러스, 독소 등 위장을 통과하면서 마주치는 수많은 이물질을 흡착해 점토 안에 가두고, 신체에 흡수되지 않고 반응성도 거의 보이지 않으며 그대로 끌어안은 독소들과 함께 배출된다.[3] 쉽게 말해 장운동 전체를 일시정지/매우 느리게 하기.[4] 드물지만 심하면 평생 부작용, 합병증, 후유증에 시달릴 수 있다. 특히 어린 아이에게 치명적이라 절대 의사의 지시 없이 지사제를 먹여선 안 된다. 요즘에는 바이러스성 설사에도 쓸 수 있는 지사제가 나오고 있지만(예: 스멕타이트 성분의 약) 설사의 원인은 매우 다양하기 때문에 무작정 쓰면 위험한 건 똑같다.[5] 주로 일본에서 직장 회식 다음날 긴 회의에 참석하기 위해 회의 전 이 약을 챙겨먹는 사례가 많은데 이 약을 먹다가 두 번째 방어에는 실패하고 회사에서 사태가 벌어졌다는 후기와 전철 안에서 지렸다는 매우 난감한 사례도 있다. 이러한 사례들은 대부분 전날 음주나 회식 등 장이 좋지 않을 때 발생한 경우라서 장의 변이 매우 묽거나 괄약근이 약할 때 발생한다.[6] 특히 소금물, 효과는 좋으나 고혈압이 있거나 당뇨병 등 신장질환을 앓고 있다면 절대 좋지 않다. 정말 골로갈 수도 있다.[7] 식이섬유가 풍부한 채소와 과일 같은 것이 좋다.[8] 과립형 하제의 경우 같이 음용할 물에 희석시켜 복용하는 식이며, 알약의 경우 물 음용 전 8~10정을 한꺼번에 복용해야 하는 식이다.[9] 다만 이 기사와는 달리 방귀에는 불이 붙을 수 있다.
