1988년 4월 18일에 싱글 발매되었다. 빌보드에서 1위를 기록했으며 벨기에에서도 1위를 했다. 영국에서는 4위, 네덜란드에서는 2위를 기록했고 독일에서는 3위를 했다.

1986년에 지어진 곡이며, Beat It에 이어서 마이클 잭슨이 록에 관심을 가지며 지은 곡이다. 마이클 잭슨은 이 곡을 사생팬의 이야기를 담은 곡이라고 했지만, 향간에서는 다이애나가 마이클이 사랑했던 다이애나 로스를 가리킨다는 견해도 있었다. 왜냐하면 이 곡을 녹음했을 당시 다이애나 로스가 마이클 잭슨의 자신과 결혼해야 한다는 바람과는 달리 다른 사람과 결혼을 선언했기 때문이다.

뮤직비디오에 출연하는 기타리스트는 빌리 아이돌로 유명한 스티브 스티븐스로 배드 앨범의 메인 기타리스트이다. 실루엣으로 등장하는 여인은 〈The Way You Make Me Feel〉의 여주인공과 동일인물인 모델 타티아나. 후에 투어 도중 미국 공연에도 참가했다가 마이클에게 기습 키스를 하고 바로 해고되었다.[5][6]

배드 월드 투어의 영국 공연 당시 다이애나 스펜서 왕세자비에게 모욕감을 줄 수도 있다며 이 곡을 리스트에서 지웠다. 하지만 오히려 다이애나 비는 "이 노래를 안 부른다구요? 세상에, 전 그 노래 엄청 좋아해요!"라고 말했다는 후문이 있다.

빌보드 핫 100에서 1988년 7월 첫째 주에 1주 1위를 기록했고 5주 연속 10위권에,[7] 8주 연속 20위권에,[8] 11주 연속 40위권에 머물러 있었다.[9] 빌보드 R&B 차트에서 8위를 기록했다.

The Weeknd가 인디 아티스트로서 데뷔했던 2011년, Trilogy 믹스테잎 시리즈의 마지막 작품이었던 'Echoes of Silence'에서 첫 번째 트랙으로 이 곡의 커버곡을 담은 바 있다. 이름은 D.D.로, 마이클 잭슨의 버전보다 더 수위를 높인 버전으로 개사해 불렀다. 위켄드가 잭슨의 음악에 보컬과 함께 깊은 영향을 받은 아티스트인 만큼, 잭슨의 느낌이 물씬 느껴지는 곡이지만 원곡보다는 더 어둡고 소울틱한 분위기로, 보컬 역시 허스키한 강렬한 락 사운드의 원곡이 아닌 잭슨의 초기 보컬처럼 팔세토를 강조한 R&B 보컬이다.

1988년 영국 공연 전에 마이클 잭슨이 다이애나 스펜서를 만났는데, 다이애나 왕세자비가 백스테이지에서 마이클 작슨에게 "오늘 'Dirty Diana'를 부를 건가요?"라고 물었다. 마이클 잭슨이 존경의 표시로 제외했다고 답하자, 그녀는 오히려 "그 노래는 내가 가장 좋아하는 노래 중 하나예요"라며 공연해주기를 요청했었다. Dirty 뜻이 문란하다, 더럽다는 뜻이래서 마이클 잭슨이 이 노래를 제외하려고 했는데 다이애나가 최애곡이라 어쩔 수 없었다.
